Home From Home Care provides specialist residential care for adults (18+) with learning disabilities, autism and complex health & mental health. We create bespoke and dynamic environments, care and support tailored to enable each person to lead a fulfilled adult life on their own terms. Now the largest family-led provider in the country, our unique approach combines the family perspective with experienced professionals from social care, creating a dynamic and outcomes-based service that continually strives to offer individuals with a learning disability better outcomes and wider horizons. We recognize and celebrate the uniqueness of every individual by ensuring a person centred approach is adopted to encourage each individual to fulfil their personal dreams and wider aspirations.

Home From Home Care has 11 homes across 5 locations in Lincolnshire, supporting individuals who are funded by over 55 Commissioning Authorities nationwide. We do not operate a 'one size fits all' approach, as we design completely bespoke packages and environments around each individuals based on their needs.

All placements and availability are dependent on our own in-depth assessment, which would be the first step and a referral for assessment would need to come from the funding authority to us in order to move forward. Our assessment enables us to determine whether we would be able to support the individual's needs and offer a suitable placement, which takes into account compatibility with other individuals and also suitability of environments, looking across all the services. Our assessment is free of charge and of no obligation to the funding authority.

We place heavy emphasis on ensuring individuals who share an environment are compatible with each other, which is not necessarily based on their diagnosis. We explore characteristics and match individuals in this way - we have learnt through experience that in doing this, individuals thrive and develop their potential to a much higher degree.

Over the years we have invested heavily in Positive Behaviour Support as a fundamental and holistic practice throughout our services. We now have a team of 20+ PBS practice leaders working cohesively across the services within the core teams for each individual, creating bespoke and sustainable strategies to support individuals, reducing behaviours of concern and enhancing quality of life.

Our high-tech, award winning, data-driven approach empowers colleagues to deliver outstanding therapeutic services, and we embrace new and innovative ways to deliver outstanding care to meet any individual’s changing needs. The majority of our services are currently rated ‘Outstanding’ by CQC, we have been awarded Platinum by Investors in People and we’re also the winners of Care Home Group 2020 at the National Care Awards and Tech for Good in the Lincolnshire Technology & Innovation Awards 2021.

As a family of a loved one with special needs, we understand how important information is when making those difficult and daunting decisions for their adult future.
