We are called Unified Care as it's all about unity; it's about all staff working together in order to support service users and stakeholders consistently, so everybody's working towards the same goal – caring from the heart.
We have three supported living homes and one residential home around the Woodgreen area in North London. We currently support 15 Adults aged between 26-64 with Autism, Learning Disabilities and Mental health. We currently have 4 voids in the supported living homes.
All homes have 24 hours support which includes a sleepin or waking night. The rota is devised according to the needs of the individuals. Each person has their own bedroom (some with En-suit) and share communal bathrooms, toilet, kitchen and lounge. One home also has a sensory room in the garden that can be used by all.
In 2019 and again in Feb 2024 we have been Accredited with the National Autistic Society and have a good CQC rating in all 5 areas.
As a provider, we provide positive behavioural support and have detailed support plans, Risk assessments and use the outcome star to show individual goals and progress made. The people we support are involved in every aspect of their lives and are at the centre of every decision. Each person we support has an individual activity plan. We also have an activities coordinator and plan regular activities with all the homes throughout the week. These activities include dance, yoga, football, cinema, meals out, trips to local parks, farms, Pedal power, arts and crafts etc.
We also have a company car so can access activities out of the area. We use many different tools to help people communicate and use their preferred method of communication. This includes the use of Pictures, photos, symbols, Makaton and objects of reference. Communication profiles are also devised to help everyone understand how best to communicate with each individual. This also indicates how each individual makes choices and decisions.
We use skills assessments to break down a task according to the individual’s ability. This teaches an individual how to become more independent and learn a new task ie changing a bed, making a cup of tea, sweeping the floor, making spaghetti bolognaise etc. This also identifies progress made or areas that require more support. We also use Outcome stars to identify goals and development. We always promote and encourage healthy living within the services and offer healthier food choices and encourage regular exercise. We also provide free Dance, yoga and football sessions each week and can arrange additional classes where a need is identified. We also offer a free holiday for the 1st year for any new tenants and go on annual holidays in the UK or abroad.
